False application schemes are operated by applicants for coverage or unscrupulous agents who make the applicant appear to be a better risk for policy acceptance. In these schemes, the operator deliberately submits false information to an insurer, government plan or other payer to obtain undeserved insurance coverage. An application for insurance requires a person to truthfully answer the questions posed on the form. A person who deliberately submits materially false information on an insurance application to obtain coverage is risking a lot.
